This Mistward armor guide is organized differently than the collection achievement screens in-game and the pages on the official wiki. I've grouped tasks into categories to make doing them a little easier, rather than grouping them by each piece of armor in the set. This will allow you to do all the champions one after another, all the masteries in order, all the insignias one after another, etc. To view the requirements for any given piece of armor in the set, you can view the specialization collection achievement for that item in-game (see graphic) or on the Mistward armor wiki page.

Champions:

Each champion below drops a collection item required for a piece of armor. The drop rate is 100%. You can view upcoming champion spawns at gw2timer.com.

The Chak Gerent event ("King of the Jungle") was generally considered near-impossible at the time this guide was written, but this seems to no longer be the case. You must complete this event for the Mistward helm, but there are several other pieces of head armor that look great with the Mistward set, including the three revenant starter helms. If you complete "King of the Jungle", you must open the chest at the end to get the Mistward Helm Casing.

Mobs:

These mobs drop a collection item required for a piece of armor. Based on my experience, it takes anywhere from 30 seconds to 20 minutes of killing these enemies for the collection item to drop.

Armor Sets:

You will need to unlock the skins for all pieces (except the head items) in the Scale, Barbaric, and Gladiator armor sets. They can be purchased on the Trading post.

Currencies for Insignias:

The following currencies are required for purchasing the insignias. You can view all (except the crystalline ore) in your inventory wallet. Crystalline ore can be viewed in your personal bank.

Vendor Acceptance (Masteries):

Three vendors require mastery in order to purchase insignias from them (i.e., three insignias require a certain level of Heart of Maguuma mastery). If you need additional mastery XP in the Heart of Thorns maps, do the daily challenges (e.g., Salvage Pit, Shooting Galley, etc) and map events. Note that a large amount of XP comes from Heart of Thorns story missions, but only as a bonus the first time you complete those missions. After that, you won't get much XP from them.

Insignias:

These are purchased from vendors. Some vendors require mastery (see above).

The meta event must be completed in Dragon's Stand (i.e., the Mouth of Mordremoth must be defeated) in order to access the Crystalline Ore Collector (vendor) and purchase the Mistward Helm Insignia. You can check the event timers for the start time of the Dragon's Stand meta event.

Mistward Headwrap

You will need all the other pieces in the Mistward armor set to complete the headwrap collection achievement.

Seems that the Chak Gerent event ("King of the Jungle") is no longer as difficult as it once was. Gabe, Jed, and I were wandering around Tangled Depths getting hero points when we stumbled onto the event. The Chak Gerent had about 60% health, and we helped complete the event. After the event ends, there's a chest (more of an obelisk, really) that spawns. You'll receive the Mistward Helm Casing when you open it.
